    POSITION SUMMARY

    The Manager of Care Coordination is responsible for oversight of services provided by the Care Coordination program, including the selection, training, and supervision of Supervisors and Coordinators. This manager will be responsible for the full success of the services in which they supervise. Assures compliance with HFS rules and all regulations for the Care Coordination and Support Organization (CCSO). The manager of Care Coordination will collaborate with the Manager of Crisis Care to ensure collaboration between programs and coordination of care for mutual clients. This position will also be responsible for community provider education on CCSO, collaboration for coordination of services, and data sharing within the designated service area. As a member of the Leadership Team, assists with the development of the program budget, organizational policy, and other administrative initiatives.

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

    • Responsible for hiring Supervisors for Wraparound Care Coordination and Intensive Care Coordination including interviewing, selecting, and training program staff. Monitors employee performance and completes quarterly and annual evaluations for direct reports. Works to resolve any performance issues or concerns.
    • Implement best practices and Evidence-Based Practices for Systems of Care and Wraparound. 
    • Assist with departmental strategic planning and collaborate with providers in DSA 2 related to care coordination for youth enrolled in Pathways to Success.
    • Maintains positive relationships and open communication with community providers and others with whom the agency collaborates on behalf of clients. Resolves any concerns regarding care coordination, linkage to services and/or engagement in services for youth/families that are enrolled in Pathways to Success in DSA 2.
    • Assures that the program metrics and requirements are fulfilled and is up to date on all State reporting requirements. 
    • Responsible for all program reviews and the success of the program's key performance indicators. 
    • Responsible for signing all IATPs as the LPHA, to confirm medical necessity for youth enrolled in Pathways to Success in DSA 2. 
    • In coordination with program supervisors, the manager shall monitor Care Coordinator caseloads monthly to ensure caseloads are balanced consistent with the caseload ratios for their assigned tier.
    • Assures that the Medicaid component of programs is fulfilled to maintain the Medicaid standards; is up to date on contractual requirements and assures compliance. Works to achieve performance goals of all contracts/grants and payer sources. 
    • Required to complete all required HFS and agency training.
    • Adheres to and follows all other required duties necessary and assigned by direct supervisor.

    SKILLS and ABILITIES

    • Strong organizational and time management skills.
    • Excellent oral and written communication skills.
    • Ability to develop and maintain relationships with community providers in the assigned service area. 
    • Skills in making public presentations, and in conducting formal training internally and externally.
    • Understanding of Systems of Care and Wraparound principles preferable
    • Interpersonal skills to work with the staff of all departments, leadership, and community organizations

    EDUCATION AND/OR EXPERIENCE

    • Required: A Master's Degree in Social Work, Counseling, or Psychology from an accredited program, or in a related human service field.
    • Required: Currently holds IL license of Clinical Social Work (LCSW) or Licensed Clinical Professional Counselor (LCPC).
    • Two years of administrative and clinical supervisory experience required.
    • Possess the experience, personal qualities and practice skills to work effectively with professionals from a variety of disciplines both internally and with other organizations.

    REQUIREMENTS

    • Must be at least 21 years of age.
    • Must possess and retain a valid driver's license.
    • Must possess and retain proof of automobile insurance.

    CARE & TCI 

    One of the essential job functions of any position within the agency is to align your performance with our overarching philosophy of CARE. All decisions, actions, communication, and performance should align with the six CARE principles (Developmentally Focused, Family Involved, Relationship Based, Ecologically Oriented, Competence Centered, and Trauma Informed). Not only must you complete the CARE training with satisfactory participation, but you must then put that training into practice in your everyday work. 

    Every employee must also pass TCI training with a satisfactory grade in written and/or physical testing. All employees must remain current on refresher training that is provided monthly at all locations. You will be tested every 6 months on the physical portion, if applicable to your position. You will be tested annually on the written portion regardless of your position. If you are not current on your TCI refresher training every six months, you will be required to retake the full course. If you do not pass either test, you may be required to retake the full course. Not only must you complete the TCI training with satisfactory participation, but you must then put that training into practice in your everyday work.
